Make a Referral

Overview

The Children’s Center of Ohio, LLC provides structured, therapeutic residential treatment for youth ages 8-18.  Referrals are accepted from county agencies, juvenile courts, and other authorized placing entities.  Our admissions team works collaboratively to ensure timely review and appropriate placement decisions.

If you know a child who may benefit from our residential treatment services, we encourage you to contact our referral team.

Referrals are reviewed Monday through Friday, and you’ll receive a timely response with our decision.

Step 2: Submit Referral Documentation

To ensure an efficient review process, please provide:

  • Completed referral information
  • Psychological and/or psychiatric evaluations
  • Social and placement history 
  • Educational records (including IEP/504 if applicable)
  • Court documentation (if applicable)
  • Current medication list
  • Risk assessment and safety plans

Step 3: Review & Assessment

Once documentation is received:

  • The clinical and administrative team reviews the referral for proram appropriateness
  • An interview (virtual or in-person) may be scheduled
  • Additional documentation may be requested if needed

Placement decisions are communicated promptly following review.

Step 4: Admission Coordination

If accepted:

  • Admission date is scheduled in coordination with the referring agency
  • Required consents and placement agreements are completed
  • Per diem billing arrangements are established with the placing agency funding source.

After Admission

  • Initial assessment are completed upon intake
  • An individualized Treatment Plan is developed within 14 days
  • Monthly treatment reviews are conducted
  • Discharge planning begins immediately to support successful transition
